NUHEALTH TO RECEIVE $3.3M FROM CMS TO SUPPORT HOSPITAL-MEDICAL HOME DEMONSTRATION PROJECT
FUNDING WILL IMPROVE COORDINATION, CONTINUITY AND QUALITY OF CARE FOR MEDICAID BENEFICIARIES BY TRANSFORMING OUTPATIENT PRIMARY CARE TRAINING SITES INTO HIGH QUALITY PATIENT CENTERED MEDICAL HOMES
East Meadow, NY……..Arthur A. Gianelli, President/CEO of the NuHealth System, is pleased to announce that the New York State Department of Health (NYSDOH) received approval from the Centers for Medicare and Medicaid to award the NuHealth System $3.3M in funding to support the Hospital-Medical Home Demonstration Project as part of the most recent extension to the Partnership Plan Section 1115 Waiver.
“This is great news for the NuHealth System. This funding will help support the operational and clinical investments required to transform our hospital-based practices into patient centered medical homes,” said Gianelli. “The purpose of the program is to encourage teaching hospitals to improve coordination, continuity and quality of care for Medicaid beneficiaries by transforming their outpatient primary care sites into high quality patient centered medical homes.”
In a recent letter, Jason Helgerson, Medicaid Director in the NYSDOH Office of Health Insurance Programs, indicated “The Department has reviewed and approved your application for participation in this initiative. The estimated award amount for the Demonstration period is $3,324,744, funds that will be distributed on the successful completion and documentation of program milestones. Funding beyond December 2013 is contingent upon the hospital achieving NCQA recognition as a level 2 or 3 Patient Centered Medical Home at all sites participating in the Demonstration.”
Gianelli added: “Special thanks to Dr. Steven Walerstein and Robert Yost for doing the necessary work to secure this funding. This latest funding complements the $18M HEAL-NY grant that NuHealth received in early summer, which will fund the construction of brand new outpatient facilities right off the lobby of the Nassau University Medical Center, as well as the $17.5M in transitional rate support which the NYSDOH recently awarded to NuHealth. Once we achieve NCQA medical home status, NuHealth will be entitled to additional per member per month funding to support the investments required to maintain this status.”
