IPRO RECEIVES NY STATE DEPARTMENT OF HEALTH HEAL PHASE 10 GRANT TO ASSIST THE NUHEALTH SYSTEM IN COORDINATING CARE FOR PATIENTS WITH DIABETES

FUNDING WILL HELP STREAMLINE AND IMPROVE QUALITY OF CARE OF CHRONICALLY ILL PATIENTS BY SUPPORTING IMPROVEMENTS IN HEALTH CARE TECHNOLOGY

EAST MEADOW, NY… Arthur A. Gianelli, President/CEO of the NuHealth System, announced that IPRO has obtained a New York State Department of Health $5.3 million grant in HEAL NY Phase 10 program (Health Care Efficiency and Affordability Law for New Yorkers), for IPRO’s medical home partners, the Queens LI Medical Group (QLIMG) and the Nassau Health Care Corporation, now NuHealth, to assist in coordinating health care of chronically ill diabetes patients.  The HEAL funding is being provided to hospitals and nursing homes to assist them in complying with the mandates of the Commission on Health Care Facilities in the 21st Century, known as the Berger Commission. The NuHealth system is comprised of the Nassau University Medical Center, the A. Holly Patterson Extended Care Facility and various family health centers, located throughout Nassau County.

“Receiving notification that our joint application with IPRO, for HEAL Phase 10 program funding for NuHealth and the QLIMG has been approved for $5.3 million for the care coordination of diabetes patients through the patient-centered medical home model, is indeed very welcome news from the State of New York. Although focused on diabetic management, this grant is a major component of our ambulatory reform strategy and is very important to the long term benefit of the NuHealth System patients, enabling us to further achieve the quality of care and improve the delivery of critical services, in a more cost-effective manner for chronically ill New Yorkers,” stated Gianelli. 


The new electronic medical records system will include continuity-of-care documentation, pharmacy information, patient health records, patient-clinician portal communications, referral and results tracking and shared community services information. This much needed technology upgrade will improve patient care and provide better service to the growing population of aging New Yorkers, by centralizing care delivery for individuals with chronic disease.”

A key project goal is to assist partner organizations in transmitting and receiving electronically shared clinical information via the Regional Health Information Exchange Organization (RHIO). The RHIO partner for the
project is the non-profit Long Island Patient Information Exchange (LIPIX). The project will promote coordinated care for more than 30,000 adults with diabetes.

Mr. Gianelli expressed his appreciation and gratitude to New York State officials and those involved in the submission of the successful joint grant application with IPRO: “We are indeed looking forward to working with IPRO, QLIMG and LIPIX on this important initiative to improve the quality of care for our diabetic patients.”
