GIANELLI APPOINTS DAVID NEMIROFF AS NUHEALTH’S VICE PRESIDENT FOR CARE TRANSITIONS

GIANELLI APPOINTS DAVID NEMIROFF AS NUHEALTH’S VICE PRESIDENT FOR CARE TRANSITIONS

–Nemiroff will oversee discharge planning process, post acute care management and navigation efforts as well as the connection of NUMC’s inpatients to appropriate outpatient primary care settings–

East Meadow, NY…….Arthur A. Gianelli, President/CEO of the NuHealth System, announced the appointment of David Nemiroff as Vice President for Care Transitions. In his new role, Mr. Nemiroff will oversee the discharge planning process, NUMC’s acute care management and navigation efforts, and the connection of the hospital’s inpatients to the appropriate outpatient primary care settings. He will also assist in developing behavioral health services into the Family Health Centers.

“David Nemiroff’s extensive background and outstanding track record of success make him the right choice for this position. This is an important role for the NuHealth System as a robust care transition program—the management of patients from the point of acute care discharge to their post acute settings—is critical to the ability of hospitals to reduce readmissions, improve mortality rates, connect patients with primary care and ensure medication reconciliation and compliance,” stated Gianelli. “There are best practices throughout the U.S. which show that investments in care transition management reduce unnecessary hospitalizations and by extension, the cost of caring for populations of patients.”

Mr. Nemiroff has over 20 years of progressive leadership experience in the Health and Human Services field.   From 2007 to 2013 Mr. Nemiroff served the Mental Health Association of NassauCounty, Inc. as Executive Director. Mr. Nemiroff shared his talents for strategic planning, administration, and strong financial and analytical skills along with his dedication to the consumers and staff.  Mr. Nemiroff worked with government, consumers, advocates and community organizations to help shape public policy to support people with mental illness and autism in the ever changing world of health and human services delivery.

Serving on the Board of Directors of the Health and Welfare Council of Long Island, Community Advocates, Mr. Nemiroff, additionally, has held board positions with the Long Island Federally Qualified Health Center, liaison with the Mental Health Association of New York State, Advisory Committee Member for the Long Island Behavioral Health Organization (LIBHM).  Mr. Nemiroff received his BS in Psychology from SUNY Oneonta and his Masters in Social Welfare from SUNY Stony Brook with an emphasis on planning administration and research.  Mr. Nemiroff lives on Long Island with his wife and two sons.

Mr. Nemiroff stated: “I am delighted and honored to have been appointed to this important position within the NuHealth System, with NUMC having a long historical mission of providing quality care to LI’s most vulnerable populations. I am honored to accept the challenge of connecting medically underserved, economically challenged residents of Nassau County to well care, with the goal of preventing the onset of chronic illness and hospitalization and addressing serious illnesses that may already be present, through prevention, education and chronic disease management.”
