Guide to Medical Health Benefits and Coverage

The Nassau University Medical Center provides health benefits to its residents. Health insurance coverage becomes effective July 1. There is a 15% contribution toward the cost of health insurance. Please see the Insurance Rate Schedule (this schedule changes every year).

Health Benefit Buyback Program
Residents who can demonstrate and attest to having other employer-sponsored group health insurance may elect to opt out of NYSHIP’s Empire Plan or Health Maintenance Organizations.

Vision Benefits

You will be automatically enrolled under Davis Vision Plan – benefit will be available December 1, 2013.

OTHER IMPORTANT INFORMATION

Be sure to have the following documents available when you attend the orientation.

  1. An original social security card
  2. Spouse’s social security number (if applicable)
  3. Visa information and authorization to train in USA
  4. Dependents’ exact birth dates
  5. Beneficiaries’ exact birth dates and addresses
  6. *Children’s birth certificates
  7. *Marriage certificate
  8. Children’s social security numbers.

Visa documentation must be included for all foreign residents. Lack of valid immigration documentation by July 1 will at the very least delay both your paycheck and your employment start date. If you do not yet have a social security number, please leave blank.

*Documents must be translated in English. These items are essential to enroll in health, dental and optical insurance plans and, most important, to receive a paycheck.
